Antiviral volgt Syd March, een medewerker van een instelling die injecties met levende virussen van zieke beroemdheden aan geobsedeerde fans verkoopt. Syd levert ook illegale monsters van deze virussen aan criminele organisaties, door ze via zijn eigen lichaam uit de kliniek te smokkelen. Als hij besmet raakt met de ziekte die superster Hannah Geist heeft geveld, wordt Syd het doelwit van verzamelaars en fanatieke fans. Hij moet het mysterie rond haar dood ontrafelen, voordat hem hetzelfde lot wacht.

Syd also supplies illegal samples of these viruses to piracy groups, smuggling them from the clinic in his own body. He must unravel the mystery surrounding her death before he suffers the same fate.
